Terms & Conditions
This document outlines the Terms and Conditions for the Community Health Scholars Program and relevant events administered through Central Valley Scholars. By submitting an application for our program, you are agreeing to these terms and conditions.

You must read these Terms and Conditions in their entirety before you accept your program offer. When you accept your program offer you are agreeing to and accepting these Terms and Conditions and agree to the requirements and conditions of your award.

Communication
All correspondence during the application process will be sent to the applicant’s primary email address submitted in the application form, with the option of telephone communication, based on Central Valley Scholars decision.
​
It is the responsibility of the fellowship recipient to inform the Central Valley Scholars of any changes to their email, postal or residential address. It is also the responsibility of the participant to inform Central Valley Scholars if they change their place of school / tertiary institute during the course of the program.
​
Any changes to the applicant's email address, postal or residential address without notice, after submission of application and without necessary cause, may result in cancelation of their application.
​
Technology and Software
If you have also been provided with technical equipment as part of your participation during the program, Central Valley Scholars does not offer any warranty period and accepts no liability for any loss or damage of the equipment.
​
Borrowed property of Central Valley Scholars must be returned in the same condition that it was received. If a student borrows a laptop they must keep the original box. Students also agree to mail laptops or other property back to Central Valley Scholars to a given address, after completion of program or if program is terminated.
​
Students are responsible for laptops or other property while they are loaned to them. If property is stolen, damaged, or in need of repairs it will be the responsibility of the student who will be held accountable for the laptop. Students must notify Central Valley Scholars staff to know if property is lost, stolen, or broken as soon as possible.

Eligibility
​​

Must be a high school student (grades 9th-12th), a community college student, or undergraduate in the upcoming 2021-2022 school year
-
Must currently reside or originate from a county in the San Joaquin Valley
-
Must be interested in community based research and health & wellness topics

Attendance
The students should notify the director if they will be absent or will be late to program sessions or events that are required. Students may be removed from the program if they miss more than 3 meetings or days of the program without notifying the program director. Removal from the program may result into less or no award.

Award Termination
Central Valley Scholars may terminate your award if you have:
​

failed to meet the eligibility requirements of your award;
-
failed to meet the program terms and conditions;

an amicable and realistic payment process cannot be reached
-
been made an incorrect award offer as determined by the Central Valley Scholars at its sole discretion; and/or
-
have provided incorrect, false or misleading information or withheld relevant information on your application or after receiving an award offer

Providing false or misleading information is an offense. If Central Valley Scholars knows or has reason to believe that you have provided false or misleading information, Central Valley Scholars may reassess your entitlement to the award and you will be liable to repay to Central Valley Scholars the total sum of any payments incorrectly made to you. If your award is terminated, you will not receive any further payments.

Repayment Of Your Award

You will need to repay the award if:
​

the payment is made to you in error or you have been overpaid;

your award is terminated
-
you do not meet the conditions of the award or are deemed to be ineligible for the award; and/or
-
you have been made a award offer in error
​
In the event that you are required to repay your award, you must do so within 4 weeks of receiving the notification.
​
How This Award Affects Your Taxes
​
Central Valley Scholars is not responsible for and cannot provide you with financial advice on taxation matters. The information provided here is of a general nature only and does not take into account your individual financial circumstances. In most cases, if you are a full-time student, your award payment may not be taxed.
Central Valley Scholars is not liable for the payment of any tax or any other obligations you may incur as a result of accepting an award from us. You must seek appropriate financial advice prior to accepting an award.

Publicity and Promotion
If you participate in the program, your details including name, school, and photograph may be passed on to media and used by the Central Valley Scholars and/or partner organizations but not limited to the Central Valley Scholars’s:

Print and electronic media

If you do not wish for your details to be passed on to the media or used for promotional purposes, you must notify the Central Valley Scholars in writing.
​
If you attend events held by the Central Valley Scholars, photos and videos of you at the event (along with your name, school and etc) may be used for promotional purposes by the Central Valley Scholars and/or our partner organizations, including but not limited to the Central Valley Scholars website, social media channels, print and electronic media.
​
It is agreed that the use of these photographs and videos shall in no way be used in any other forum other than official Central Valley Scholars business. If you do not wish for photographs and videos of you to be used in this way, you must notify the Central Valley Scholars in writing.

Media Consent
Consent is sought for use of photographic/video/audio recording made of the student during Central Valley Scholars events. Permission is being requested to publish, reproduce and communicate images and / or recording of the student on:

The website and other promotional material related to the Central Valley Scholars;

In related media and other locations to promote the awards;

On websites/promotional material of event sponsor;

This consent is for an indefinite period of time. Consent is also sought to publish the name and school of the student. With reference to the above, the student grants permission to use, reproduce, distribute, communicate to the public, publish, publicly display, modify, translate, adapt, upload, download in any form or manner and / or permission to incorporate this material into other materials or works in any format or medium for any non-commercial purpose and the right to sublicense those rights for noncommercial purpose. The student can withdraw their consent at any time but must do so in writing or by email to contact@centralvalleyscholars.org.

Liability Waiver
In participating in the program with Central Valley Scholars, you agree to WAIVE, RELEASE, AND DISCHARGE the Central Valley Scholars, its officers, associates and partner organizations from any and all liability, including but not limited to, any liability arising from the negligence or fault of the entities or persons released, for death, disability, personal injury, property damage, property theft, or actions of any kind which may occur to you during or after the the program, including during or after any travel undertaken as part of the program.

Please note, if you are under 18 years of age, it is understood that your parent or guardian has read and agreed to the Community Health Scholars Program Terms and Conditions on your behalf.
​
The Central Valley Scholars reserves the right to amend these Terms and Conditions for commencing and continuing program participants.
